The medical spa industry has exploded in recent years, and for good reason — med spas offer a middle ground between day spas and surgical clinics, providing medical-grade treatments in a comfortable, spa-like setting. But not all med spas are created equal, and understanding what to look for before your first visit makes the difference between a great experience and a regrettable one.
What is a med spa?
A medical spa (med spa or medspa) is a facility that offers minimally invasive cosmetic treatments under the supervision of a licensed physician. Unlike a traditional day spa that offers facials, massages, and relaxation services, a med spa provides treatments that penetrate the skin or alter the body's tissue — procedures that require medical oversight.
Common med spa treatments include Botox and other neuromodulators, dermal fillers, chemical peels, laser treatments (skin resurfacing, hair removal, pigmentation correction), microneedling, CoolSculpting and body contouring, and medical-grade skincare.
What to look for in a med spa
The most important factor is medical oversight. Every reputable med spa should have a licensed physician — typically a dermatologist, plastic surgeon, or other qualified MD — as the medical director. This physician is responsible for establishing treatment protocols, overseeing clinical staff, and being available for complications.
Ask who the medical director is. Ask what their specialty is. Ask whether they are on-site or available by phone. A med spa without clear medical oversight is a red flag.
Next, check the credentials of the person performing your treatment. In most states, injectables can be administered by physicians, nurse practitioners (NPs), physician assistants (PAs), and registered nurses (RNs) with appropriate training. Laser treatments may have different staffing requirements depending on the state. Ask your provider about their specific training, certifications, and how many procedures like yours they've performed.
Questions to ask at your consultation
A responsible med spa will always offer a consultation before any treatment. During that consultation, ask what specific product or device will be used and whether it's FDA-approved or FDA-cleared for this indication, what results you can realistically expect, what the risks and potential side effects are, what the recovery time looks like, how many treatments you'll need, and the total cost including any follow-up treatments.
Be cautious of any provider who guarantees specific results, pressures you to commit immediately, offers prices that seem dramatically below market rates (this can indicate diluted products, counterfeit products, or inadequately trained staff), or cannot clearly explain the risks of the procedure.
What to expect on your first visit
Most first visits begin with a consultation, even if you've already booked a treatment. The provider will assess your skin, discuss your goals, review your medical history (including medications, allergies, and prior treatments), and recommend a treatment plan.
For injectable treatments like Botox and fillers, the procedure itself typically takes 15–30 minutes. You may experience mild redness, swelling, or bruising at injection sites. Most patients return to normal activities immediately.
For laser treatments, the provider will explain pre-treatment preparation (which may include avoiding sun exposure and certain skincare products) and post-treatment care. Some laser treatments require numbing cream applied before the procedure.
The bottom line
A great med spa combines medical expertise with a comfortable patient experience. Do your research, ask questions, and choose a provider whose credentials, communication style, and approach make you feel confident. The right provider will welcome your questions — not rush past them.
